We ask you to fill in this form and sign it with your electronic identity card. You can then send it by clicking on the relevant button at the bottom of the form.
The document will then be automatically added to an e-mail to the Radio Amateur Service. If necessary, you can also attach other documents to this e-mail.
If you do not have an eID card reader available, you may fill in the form electronically and email it to ram@bipt.be together with a manually signed scanned version.
You need Acrobat Reader to fill in the form. This programme can be downloaded free of charge from the Internet.
